Olopatadine, Ketotifen, Alcaftadine
Dual-action antihistamines combine competitive blockade of histamine receptors with intrinsic stabilization of mast cells, providing rapid improvement of pruritus and conjunctival edema in allergic conjunctivitis.
Mechanism
💊 Commercial NamesOpatanol, Patanol, Zaditen Ofta, Lastacaft, Ketotifeno Kern, Olopatadina Alcon.
🔬 Pharmacological GroupSelective antagonists of receptor H1 histamine and mast cell membrane stabilizers. Pyridine and dibenzoxepin derivatives.
🧪 Standard FormulationsOlopatadine hydrochloride 0.1% and 0.2% (single-dose and multidose), ketotifen fumarate 0.025%, alcaftadine 0.25%.
Mechanism of Action
These dual-action agents modulate the immediate and late phases of the allergic cascade in the conjunctival tissue:
- Selective Antagonism of Receptor H1: They competitively bind to histamine receptor H1 expressed in the endothelial cells of conjunctival vessels and in the nociceptors of free nerve endings. This instantly stops vasodilation, increased capillary permeability (chemosis) and intense conjunctival itching.
- Mast Cell Stabilization: They directly inhibit the opening of voltage-gated calcium channels in the mast cell cell membrane, preventing the influx of extracellular calcium necessary for the polymerization of microtubules and the subsequent exocytosis of secretory granules that contain preformed allergic mediators (histamine, tryptase, chymase).
- Inhibition of the Late Phase of the Allergic Reaction: They reduce chemotaxis and local activation of eosinophils and neutrophils, and stop the conjunctival release of proinflammatory cytokines such as ICAM-1, IL-4, IL-6 and IL-8, blocking the chronicity of the allergic process.
Pharmacokinetics
Quantitative Ocular and Systemic Pharmacokinetics
| Parameter | Olopatadine 0.2% | Ketotifen 0.025% | Alcaftadine 0.25% |
|---|---|---|---|
| Onset of Relief | Immediate in 10 - 15 minutes. | Immediate in 15 minutes. | Immediate in 3 minutes (The fastest in his group). |
| Duration of Effect | Up to 12 - 24 hours (allows dosage of 1 dose per day). | Up to 12 hours of dural symptomatic relief. | Up to 24 hours of conjunctival allergic protection. |
| Clearance and Metabolism | Low systemic absorption; it is excreted mainly unchanged by the kidney. | Inactive secondary phase I hepatic metabolism. | Metabolism by demethylation to prolonged secondary active metabolite. |
| Half-life (t1/2) | t1/2 in plasma ≈ 3 - 4 hours. | t1/2 plasma ≈ 12 hours. | t1/2 plasma ≈ 30 minutes (metabolite: 2 hours). |
Security
Contraindications
- Absolute: Documented hypersensitivity to the active salt or any of the components of the eye drops.
- Relative: Use of soft contact lenses during instillation (the preservative benzalkonium chloride is absorbed into the hydrogel of the lens, causing irreversible discoloration of the lens and inducing delayed corneal epitheliotoxicity; it should be advised to remove the lenses and wait at least 15-20 minutes before inserting them).
Adverse Effects (ADR)
- Ocular Sites: Mild transient burning or stinging sensation post-dosage; dry eye due to slight secondary muscarinic blockade activity; superficial punctate keratitis; transient tearing and blurred vision.
- Systemic: Mild tension headache, asthenia, dryness of the nasal mucosa and mild hyposmia. No or negligible central sedation compared to classic first-generation systemic antihistamines.
